Lutheran SeniorLife to Participate in “Phase 2” of Connect-Home Project Posted on August 16, 2019August 27, 2019 by zap Thanks to the Harry & Jeanette Weinberg Foundation, Lutheran SeniorLife and three other Lutheran Senior Services providers will participate in “Phase 2” of the Lutheran Services in America, or (LSA), project – Connect-Home. LSA implemented “Phase 1” of the Connect-Home model in collaboration with Lutheran Services Carolinas, National Lutheran Communities and Services (NLCS), Lutheran Social Ministries of New Jersey and Samaritas in 2018. As the project enters 2019, LSA will use three of the four original players to guide Lutheran SeniorLife, Genacross, Niagara Lutheran Health System and Luther Manor through the fourstep transitional care process of “Phase 2”. This expansion will allow more Lutheran Senior Services providers who desire to scale and replicate Connect-Home to manage patient illness at home, improve function, and prevent avoidable re-hospitalizations through an evidence-based transitional care process.
