Home Health Care and Hospice

Does your loved one need a little extra help living at home? Lutheran SeniorLife offers Home Health, Health Care, and Hospice services through the Visiting Nurses Association and BeHome Partners®.

Home Health Care
An independent lifestyle in the comfort of your own home is the goal of Visiting Nurses Association’s home health program. For homebound individuals with new or chronic health issues, VNA can provide skilled services on an intermittent basis to meet your needs and your personal goals while keeping you out of the hospital. The VNA commitment to Abundant Life® through health, wellness and social services, means families can live each day with purpose and meaning.

Hospice
Hospice care is the provision of physical, emotional, and spiritual care to those facing a life-limiting illness. Compassionate hospice providers discuss and explore individual healthcare options allowing participants to make decisions to remain comfortable and reach their personal goals. Earlier admission to hospice has improved quality of life and affords the blessing of more quality time with loved ones.

Services are available in the following counties Armstrong, Beaver, Butler, Lawrence, Mercer, and northwest Westmoreland.

Outpatient Therapy

Whether you’re recovering from surgery, dealing with a chronic health condition or trying to prevent injury, your health care team may recommend you receive outpatient therapy. We make it easy to get the care you need, with on-site programs, including:

Adult Day Care

Our adult day care programs give seniors living at home the opportunity to get out and spend time with others. Participants enjoy activities such as exercise classes, educational presentations, entertainment, pet visits and more. Programs also include:

  • Nursing services
  • Medication administration
  • Bathing assistance in a safe, ADA-compliant setting
  • Personal care assistance in private, handicapped-accessible bathrooms
  • Coordination of rehab services (physical, occupational and speech therapy)
  • Healthy breakfast, lunch and snack
  • Podiatry and salon services

Senior Centers

We offer two vibrant senior centers, run by the Lutheran Service Society. Designed for adults over 50 who want to have fun, stay healthy and be active, these centers provide most activities at no charge, although donations are welcome. Participants can enjoy the following:

  • Nutritious lunches
  • Classes and workshops (exercise, dance, arts & crafts, computers, wellness)
  • Group trips
  • Flu shots
  • Farmer’s Market vouchers
  • Safe driving classes
  • Income tax assistance
  • Apprise Health Insurance counseling

LIFE VieCare (Living Independence for the Elderly)

Helping seniors remain independent and safe in their homes.

             

Living Independence For the Elderly (LIFE) is an all -inclusive program of medical and support services to help people maintain independence in the community.  Nationally known as the Program of All-Inclusive Care for the Elderly (PACE), this model has proven success throughout the United States. Lutheran SeniorLife offers the Life Programs for income-qualified, older adults at the following counties:

  • LIFE Armstrong
  • LIFE Beaver
  • LIFE Butler
  • LIFE Lawrence

Meals on Wheels

Adults of all ages can receive Meals on Wheels for nutrition, safety, and convenience. Each meal delivery includes a friendly smile and a wellness check, which—especially for our elderly and differently-abled participants—alleviates isolation and provides peace of mind.

Services include:

  • Up to 14 meals delivered each week
  • Wellness check
  • Meals delivered at serving temperature upon request

Adoption and Foster Care

If you’re a family that is interested in adopting or fostering a child, you can find the help you need through the Lutheran Service Society. With 100 years of experience in western Pennsylvania, this organization is licensed and ready to support you in your journey of adopting domestic infants, older American children, children with special needs and international children. Services include:

  • Guiding families through the adoption process with a free orientation
  • Providing support services after placement
  • Offering comprehensive training and important resources
  • Post permanency support for families experiencing difficulties
  • Child preparation
  • Child and family profile development

SilverSmart Technology

The SilverSmartTM program is all about seniors utilizing technology to remain independent, safe, healthy and connected.

Some SilverSmartTM Technology items include:

  • Amplified phones, large-button phones, closed captioned phones for better communication
  • Personal sound amplifiers, voice amplifiers to improve socialization
  • Fire prevention systems, digital door views and signalers for safety
  • Magnifiers and automated devices to assist with hobbies
  • Monitors, sensors, telecommunication devices for medical assistance
