Community Life Specialist

Job Type: Full-Time

Location: Passavant Community

The Community Life Specialist organizes and implement individual and group activities and special programs. Acts as a community resource contact to organize and supervise outside groups and entertainment. Establishes appropriate goals for individual residents. Attends and participates in resident care planning. Bachelor’s degree from four-year College or university and one to two years related experience and/or training or equivalent combination. 

Community Life Technician

The Community Life Technician assists in group actives, special programs, and engaged residents in daily living activities. Documents resident attendance and participation in actives and supports Household life by participating in resident and household activities. High School diploma or general education degree (GED) needed. Six months to one year related experience and/or training or equivalent combination.

Certified Nurse Aides

The Nurse Aide provides residents with assistance in Activities of Daily Living. CNA responsibilities include the primary physical care, safety and emotional well being of residents. Current PA certification required. 

Day Center Manager, LPN

Job Type: Full-Time

Location: Valley Care - Ambridge

This LPN manager will provide oversight of staff in the Adult Day Center in Ambridge. Position monitors staffing ratios, develops and monitors shower schedules, toileting schedules, provides direction and oversight to center staff. Must have a current LPN license in PA, at least 1 year of management experience, and at least one year of experience working with the frail elderly. Must also possess excellent communication skills, time management skills and computer knowledge. 

Transportation Manager

Job Type: Full-Time

Location: LIFE Lawrence County

Responsibilities include assuring the safe and efficient transportation of participants in a timely, competent manner, and communicating important information to the multidisciplinary team. Position requires daily coordinating and scheduling of participants in Lawrence County to and from Day Center, appointments, tests, and outings. Other responsibilities include driving the van, scheduling appointments, van maintenance, record keeping, and other duties as assigned. High school diploma or GED required. Three years’ experience driving handicap vans, and experience with complex scheduling. Must have extensive knowledge of the Beaver county road system. One year of experience with the geriatric population. Must be detail oriented and possess the ability to prioritize assignments. Must have computer experience and excellent organizational, writing, and oral communication skills. Must be flexible.

Resident Assistant

Schedule: 10:30 p.m. – 7:00 a.m.

The Resident Assistant enhances the Abundant Life of residents by providing assistance through performance of routine direct and indirect resident activities according to established policies and procedures. Maintains resident rights, privacy and confidentiality. Promotes resident acceptance of service plans and responds promptly to overall resident needs.

Accountant III

Responsible for maintaining and updating general ledger master file, consolidated financial statements, cash flow statements, supporting reports and graphs. Initiates general ledger closing process, produces and reviews month end closing reports and begins analytical process to determine validity and quality of reports. Outstanding analytical skills and excellent written and verbal communication skills required. Must be self-directed and effectively manage multiple deadlines. Proficiency with spreadsheets and word processing software required. Bachelor's Degree in Accounting; and four to six years related experience; or equivalent combination of education and experience. Accounting experience in long term care preferred.

Registered Nurse Assessment Coordinator (RNAC)

Job Type: Full-Time

Location: St. John Specialty Care Center

Responsible for the management of the Resident Assessment Instrument (RAI), Minimum Data Set (MDS), process ensuring the accurate and timely completion and electronic submission of the required data; auditing documentation to ensure accuracy of the data submitted; coordination of the Interdisciplinary Team for development and implementation of care plans based on data collected and submitted; assessment of residents’ conditions to ensure proper documentation and Medicare/Medical Assistance benefit utilization. Qualified applicant must be a R.N., and have 6 months to a year related experience.

To recommend, install, maintain computer hardware, operating systems, networks and basic computer software such as Windows, and other systems as appropriate. To recommend vendors and purchases as appropriate for computer hardware and software. Associate's degree (A. A.) or equivalent from two-year college or technical school; or six months to one year related experience and/or training; or equivalent combination of education and experience. Computer maintenance experience. Working knowledge of PC's and networking.
