Community Care Coordinator LPN

Job Type: Full-Time

Enhances the abundant life of participants by providing assistance through performance of routine direct and indirect participant activities according to established policies and procedures. Promotes participant acceptance of care plans and responds promptly to overall participant needs. Participates in the hiring, coaching, disciplining, evaluation and termination of employees. Participates and contributes to the development of the annual budget for the community facility. Adheres to budgetary guidelines. Enhances Abundant Life while encouraging participant’s independence with activities of daily living. Assists participants in their self-care initiatives to include, but not limited to, bathing, dressing, undressing, grooming, toileting, changing incontinence products, medication reminders, ambulation or position changes. Enhances Abundant Life while encouraging participant independence with instrumental activities of daily living, such as laundry, keeping appointments, coordinating transportation, using a telephone, caring for personal possessions, engaging in social and leisure activities, using a prosthetic device and resolving conflicts or grievances. Must be an LPN in good standing with at least one year of management experience.

Lutheran Service Society Director (LSS) Bellevue, PA

Lutheran Services Society, part of the Lutheran SeniorLife organization, is seeking a full-time Director for its Bellevue location. The Lutheran Service Society Director will provide operational management for Lutheran Service Society’s community-based senior housing facilities, programs and community services including, but not limited to: senior centers, Meals on Wheels program, and residential facilities enrolled with HUD (Housing and Urban Development). The Director will participate in planning, organizing and directing the overall services and financial operations of this division, operating throughout a diverse geographic region in western Pennsylvania. This position will facilitates a team-oriented environment, and lead the delivery of all aspects of a comprehensive program to ensure licensure and contract compliance, and initiatives to improve service.

 

Bachelor’s degree in related field. Five or more years of management experience. Two years’ experience working in a community based service industry coordinating a network of regional programs preferred. Demonstrated effective leadership and team management to insure program growth and financial sustainability required. Knowledgeable of community based programs, service enriched housing programs and has demonstrated the ability to coordinate supportive services through third party providers. Demonstrated ability to work with and communicate effectively with community support services and business leaders. Demonstrated ability to establish and develop business and operational assessments, establish performance metrics and adhere to operational benchmarks.

Work Control Coordinator

Job Type: Full-Time

The Work Control Coordinator provides support to the daily operations of the Facilities Management department by coordinating several aspects of operations, including the work order and purchasing processes. The Coordinator creates and manages reports and filing systems. Provides excellent customer service to residents, staff and families in a timely manner. Performs daily data entry in to the work order and preventative maintenance systems. Develops various computerized reports as requested. Develops, updates and maintains tracking systems (catalogues as necessary) for the purchase of parts and equipment inventory. Tracks and reports on labor hours. Manages customer and work requests and incoming and outgoing mail.

The Production Assistant prepares all starches, vegetables, soups, salads, and/or baked goods as assigned. These food items are served to the Residents and in the Staff Cafeteria, plus for all special functions as requested.

Health Care Coordinator (RN)

RoseCrest Assisted Living Residence, part of Lutheran SeniorLife, is seeking a full time Health Care Coordinator for our 30-bed  esidence for memory impaired seniors on the St. John Community campus. Responsibilities include the clinical oversight and management of resident care for the residence including resident assessment, coordination of health care needs, training and supervision of direct care staff, regulatory compliance and customer relations. Qualified applicants will be a licensed Registered Nurse in the state of PA and a minimum of two years of experience in the medical field with experience working with frail and memory impaired residents. 

Driver Assistant

Home Care Assistant

Job Type: Full-Time, Part-Time

Location: LIFE Lawrence County

Responsibilities include performing basic household duties to assist participants to maintain an environment consistent with their needs. Provides for needs in the home and assists participants with activities such as bed making, changing linens, grocery shopping, light cleaning and light meal preparation, and trash disposal. May also be assigned to work in the Center as needed.  High school diploma or GED. One year experience with the geriatric population. Must be able to work with culturally, economically, and educationally diverse populations and form positive interpersonal relations in dealing with a wide range of staff and participants. Must have a valid PA Driver’s License and reliable transportation.

Adult Day Program Manager

Job Type: Full-Time

Location: Passavant Community

The Adult Day Program Manager is responsible for the day-to-day operations of the day center and coordination of all care delivered to participants. This includes overall planning, development, organization and progressive direction of the operations. The Manager serves as the catalyst to the achievement of continuous quality improvement in all areas of the Program and demonstrates leadership skills in, but not limited to, the following areas: recruitment, staff development, system analysis, participant centered services, management of staff and partnering with service providers.

 

LPN Required. Bachelors’ degree preferred. Four or more years of health care experience as a manager in a long term care facility, managed care, community based or outpatient health care setting. Must understand clinical operations in a capitated reimbursement environment. Must have one year experience with the frail or elderly population. Dementia experience preferred.

Floor Care Technician

The Floor Care Technician will perform a variety of job functions to ensure a welcoming, clean and safe environment. May be assigned to different areas to complete routine program and project cleaning. Responsible for the care of all types of floors and other project cleaning as required.

The Human Resources Generalist is responsible for coordinating HR management activities, such as employment, compensation, employee relations, benefits, training, performance appraisal, HRIS, and employee services. Understands and interprets HR policies and procedures and interprets the intent, spirit, and terms of each to employees and managers. Explains company and governmental rules, regulations and the need for compliance to employees and management. Bachelor's degree in Human Resources Management, Personnel or Labor Relations and three to five year’s experience or equivalent combination of education and experience.
