Eligibility

Clients must be:
• 60 years of age or older
• Certified to need nursing home level of care
• Residing in Beaver County
• Able to live safely in the community with LIFE's services at the time of enrollment
• Eligible for Medicaid, or have the ability to pay privately

To apply at LIFE Beaver County, contact the Intake Coordinator at 724-378-5400. The Intake Coordinator will explain the program and assist with completing the application process. There is no fee for the application process. There is no fee for the program if you meet eligibility requirements for Medicaid.

The application process includes a review of your health situation and financial information for eligibility purposes as well as an initial evaluation of your needs.
