LIFE Beaver County

LIFE Beaver County is an innovative alternative to nursing home care designed to restore and preserve adult independence. LIFE (Living Independence for the Elderly) emphasizes quality of life for those older individuals who wish to remain independent or who choose to live with family members.

Through LIFE Beaver County, eligible clients have a full range of medical, social and rehabilitation services available to them:

Adult day care
Support care in the client’s home
Prescription coverage
Meals
Recreational programs
Transportation to and from the LIFE Center where medical and social needs can be met.

Even if a client requires hospital or nursing care, LIFE Beaver County manages, arranges and monitors the individual's plan of care. This program makes life easier for caregivers and supports family efforts to provide care for loved ones. LIFE Beaver County helps eligible older adults who are living at home by managing their complex medical, functional and social needs. Services can be provided at the LIFE Center, in the client's home, through outpatient and specialty services and inpatient settings when necessary.
