Eligibility - LIFE Lawrence County
A person is eligible for LIFE Lawrence County if he or she is:
- 55 years of age or older
- Having difficulty managing his or her own health-related needs independently at home and meets a level of care determined by the Lawrence County Area Agency on Aging/Challenges
- Eligible for Medicaid or able to pay privately
- Living in Lawrence County
- Able to live safely at home with our services
To apply, call 724-657-8800. The Intake Coordinator will explain the program and assist with completing the application process. There is no fee for the application process, and there is no fee for the program if you meet eligibility requirements for Medicaid. The application process includes a review of your health situation and financial information for eligibility purposes as well as an initial evaluation of your needs. LIFE Lawrence County provides services and referrals to individuals regardless of race, color, creed, religion, sex, nationality, ancestry, age, sexual preferences, handicap or disability.
