Eligibility - LIFE Beaver County
A person is eligible for LIFE Beaver County if he or she is:
- 55 years of age or older
- Having difficulty managing his or her own health-related needs independently at home and meets a level of care determined by the Beaver County Office on Aging
- Eligible for Medicaid or able to pay privately
- Living in Beaver County
- Able to live safely at home with our services
To apply, call 724-378-5400. The Intake Coordinator will explain the program and assist with completing the application process. There is no fee for the application process, and there is no fee for the program if you meet eligibility requirements for Medicaid. The application process includes a review of your health situation and financial information for eligibility purposes as well as an initial evaluation of your needs. LIFE Beaver County provides services and referrals to individuals regardless of race, color, creed, religion, sex, nationality, ancestry, age, sexual preferences, handicap or disability. LIFE Beaver County is a member of the National PACE Association, www.PACE4you.org.
