Community HealthChoices (CHC) is Pennsylvania's new Medicaid managed care program.
CHC is a mandatory program for people age 21 and over who receive both Medicare and Medicaid, or those who receive Medicaid Long-Term Support Services because they need the level of care provided in a nursing facility or through one of the home and community-based waivers.
Those affected MUST enroll in Community HealthChoices by November 13, 2017 and select one of three approved plan choices. These include:
- AmeriHealth Caritas
- PA Health and Wellness
- UPMC Community HealthChoices

If you do not select one of these providers, one will be selected for you. Coverage begins January 1, 2018.
If you currently reside in a nursing facility: Once enrolled, CHC will pay the facility for the care provided. It will also provide Medicaid physical health coverage. If you receive services that are not provided by the nursing facility, you will use your CHC Plan ID card and the CHC plan will then pay the medical providers. For long-term care services and supports received in the nursing facility, the CHC plan will pay the nursing facility.
HOWEVER, if you are currently enrolled in a LIFE Program (LIFE Armstrong County, LIFE Beaver County, LIFE Butler County, LIFE Lawrence County), you DO NOT have to enroll in Community HealthChoices, as the LIFE Programs are an approved provider. Current enrollees do not need to take any action, and all services and programming will continue without interruption.
